Nurse-Led Care, Built Around Home

Acuity Meadows grew out of real-life experience and the realities families like ours face. As my own daughter, who has an intellectual and developmental disability, approaches adulthood, I, like so many parents, am filled with questions: Who will care for her if I’m not here? Will she ever be able to live independently, or have the support she needs to live on her own someday? These are the anxieties that drive our mission and the foundation of this agency.  Rather than letting fear paralyze me, I chose to create a solution. Acuity Meadows is my answer: a Program Approved Service Agency (PASA) providing a permanent, supportive ecosystem of care, so that my daughter—and others like her—can have the support they need to live as independently as possible. Our mission is to ensure responsibility doesn’t automatically fall on siblings or other family members, and that no one is lost in the system. Clinical leadership truly sets Acuity Meadows apart. Through the experience I gained from participating in the CNA Parent program—which allows parents to become Certified Nursing Assistants to care for their own children with disabilities—I advanced my career and became a Registered Nurse. Now, I lead this agency with that expertise. RN oversight means our policies and practices reflect the highest standards in health assessment, risk management, and compliance. This level of clinical leadership is exactly what state auditors, case managers, and families look for. Why We Built Acuity Meadows

Too many adults with intellectual and developmental disabilities are placed in settings that prioritize convenience over belonging. We believe every person deserves a home — not just a placement.

Acuity Meadows was founded to fill a specific gap: a Host Home agency with genuine nurse-led oversight. That means clinical accountability, proactive health management, and a team that treats each individual as a whole person — not a case number.

We are currently finalizing our PASA certification with CDPHE and HCPF. We are building this agency the right way — with the infrastructure, training, and values that will serve individuals and families for years to come.

Built to Meet Colorado's Standards

Acuity Meadows is structured from the ground up to operate within Colorado's HCBS waiver system as a fully certified PASA.

Colorado PASA — In Progress

We are actively completing Program-Approved Service Agency certification with CDPHE and HCPF. Our policies, procedures, and infrastructure are being built to meet and exceed state requirements.

Host Home Model Specialty

Unlike generalist agencies, Acuity Meadows is built specifically for the Host Home model. Every process, training, and support structure is designed around this service type.

We are actively completing PASA certification with CDPHE and HCPF. We welcome inquiries from case managers, families, and prospective host home providers while we finalize our certification.
