Our intake, documentation, and service delivery are designed to align with Colorado's HCBS waiver requirements from day one — minimizing administrative friction for you and your clients.
Pre-Launch Transparency
We are currently finalizing PASA certification with CDPHE and HCPF. We are building our infrastructure the right way and welcome early conversations with case managers who want to understand our model.

How to Make a Referral
Reach Out
Contact us by email or through our inquiry form. Share a brief overview of your client's needs, goals, and current situation. There is no formal referral packet required at this stage.
Initial Conversation
We'll schedule a call to discuss your client in more detail — their support needs, preferences, and whether Host Home with Acuity Meadows is likely to be a good fit.
Intake & Assessment
If we move forward, we conduct a thorough intake assessment — reviewing the individual's support plan, health history, and goals to inform the matching process.
Matching
We identify provider families whose household, capacity, and personality are compatible with your client. We share information about potential matches and involve you and the individual in the decision.
Placement & Transition
We support a gradual, thoughtful transition — including visits and trial stays. Once placed, we provide ongoing nurse-led oversight and keep you informed throughout.
