The Host Home model places adults with intellectual and developmental disabilities in the private home of a caring provider family — not a group facility. It's one of the most person-centered living options available under Colorado's HCBS waiver system.

What is Host Home?
Living With a Family, Not in a Facility
In a Host Home arrangement, a certified provider family opens their home to one or two adults with IDD. The individual lives as a genuine member of the household — sharing meals, participating in community activities, and building real relationships.
This is fundamentally different from a group home or residential facility. There are no shift changes, no institutional routines, and no revolving staff. The provider family is present, consistent, and invested in the individual's wellbeing.
Acuity Meadows oversees each placement with nurse-led clinical support — ensuring health needs are monitored, providers are trained and supported, and individuals receive the level of care their plan requires.

The process
How a Host Home Placement Works
Initial Inquiry
A case manager, family member, or individual reaches out to Acuity Meadows to explore whether Host Home is the right fit. We discuss the individual's needs, preferences, and goals.
Matching
We carefully match individuals with provider families based on compatibility — shared interests, household environment, location, and support capacity. Good matching is the foundation of a successful placement.
Provider Preparation
Our provider families complete thorough training and background screening before any placement. We ensure they have the skills, knowledge, and support to provide excellent care.
Transition & Move-In
We support a gradual, thoughtful transition — including visits, trial stays, and ongoing check-ins — so the individual and provider family can build trust before the full placement begins.
Ongoing Support
Acuity Meadows provides continuous nurse-led oversight, regular home visits, provider support, and care coordination throughout the placement — not just at the start.
